• Energy concession - A copy of your most recent electricity bill (or LPG gas receipts) or other evidence of your energy use or charges.
• Water and sewerage rate remission - Your most recent water bill from SAWater or other water supplier.
• Council rate remission - Your latest annual council rates notice (showing the total rates amount for the year).

 *Please note: there is a separate form to apply for the Emergency Services Levy concession.

Fill in the form
Fill in the form by entering the appropriate information into the spaces provided. Ensure you fill in all of the spaces and sections of the form. The information you are required to provide may vary, depending on the information you have provided in previous spaces or sections.

Print the form
If you are submitting your application on line, PRINT the completed form and keep it for your own records.

Submit 
Submit your application online by clicking on the grey button marked ‘submit application’, or, if you prefer, print out your application and mail it to us at the address indicated on the form.

Record your receipt number
If you have completed and submitted your form on-line, after a few moments, an acknowledgement of your application will appear on screen showing a receipt number. We recommend that you print the receipt or write down the receipt number and keep it for your records. Please quote this receipt number if you contact us with any queries or if you need to send us any supporting documentation.

Send in any supporting documents (if required) 
Your receipt will indicate whether you need to send in any supporting documents with your application, such as a copy of an account or receipts for purchasing water or energy.
